Naomi House

Naomi House accepts referrals for babies and children from 0-16 years of age from across the central south of England, including the counties of Berkshire, Dorset, Hampshire, Isle of Wight, Surrey, West Sussex and Wiltshire.

Referrals to Naomi House can be made by parents/carers or by a professional involved in the child’s care, with the parents/guardian’s permission.

We will ask for some initial information regarding diagnosis and current health concerns, and confirm parental consent to access further supporting information from the professionals involved.

Once all information is received, the referral will be assessed at our Clinical Committee using our Eligibility Assessment Tool, and a decision made regarding access to our services. We will then contact family and professionals with this decision.

Jack's Place

Jack's Place accepts referrals for young adults 16-35 years of age from across the same area. Referrals for Jack's Place can be made from outside these regions, but we will seek statutory funding to support your care. 

Referrals can be made to Jack's Place by yourself, or by a professional involved in your care.

The process for this involves taking information regarding diagnosis and symptoms and gaining your consent for the information to be processed. We then write out to your professionals for their views and supporting information for the referral.

Once all information is received, the referral will be assessed at our Clinical Committee and a decision is made regarding access to our services. The decision is then communicated to you and the medical professionals involved in your care.
